Card Payment Policy
We use Square Stripe for secure auto-billing. Your card information is stored safely for future appointments, package renewals, recurring bookings.
Insurance Policy
We do not process insurance claims, superbills, invoices, or medical billing codes. For personal injury or legal cases, records can be provided directly to your attorney upon request.
Common Therapy CPT Codes
- 97124 – Massage therapy (effleurage, petrissage, tapotement, etc., one or more areas, each 15 minutes).
- 97140 – Manual therapy techniques (mobilization, manipulation, manual lymphatic drainage, manual traction, etc., each 15 minutes).
- 97110 – Therapeutic exercises (strength, endurance, ROM, flexibility; 15 minutes).
- 97530 – Therapeutic activities (functional performance activities; 15 minutes).
- 97112 – Neuromuscular reeducation (movement, balance, coordination, posture, proprioception; 15 minutes).
Foot Levelers Orthotics Policy
Custom Foot Levelers orthotics are available only to active patients and are non-refundable.
Warranty Information: All products come with a 1-year warranty, except for flip-flops, which have a 90-day warranty. Custom pillows include a lifetime warranty.
Bodywork Maintenance Program Policy
Patients are responsible for managing their own schedules. Missed appointments without notice will not be rescheduled. A 24-hour notice is required for cancellations. Written notice must be provided at least 30 days before or after minimum program requirements to end recurring payments.
Luxury Spa Treatment Policy
All luxury spa treatments require advance payment. A minimum of 48 hours’ notice is required for cancellations or reschedules to avoid penalties.
Cancellation No-Show Policy
You can cancel or reschedule your appointment online up to 24 hours prior to your scheduled time. Same-day cancellations or missed appointments are subject to a no-show fee of up to 100% of the scheduled service. Adult patients are responsible for managing their own cancellations, even if a parent or guardian makes the payment for patients over 18 years of age.
Refund Policy
Refunds are not available as we provide professional therapeutic services rather than physical products.
Patient Responsibility Policy
Patients over 18 years old are responsible for managing their appointments. Parents or guardians who pay for adult patients are not responsible for cancellations or rescheduling.
